Return each push notification type a provider can turn on or off, with whether the provider sends it. A type with no stored choice reads its default. Also returns the missing forms & waivers push schedule.

📄️Partially Update

Turn push notification types on or off for the provider, and set the missing forms & waivers push schedule. Only the types and fields in the request change; the others keep their values. All the changes are saved together, or none are.
